In theory it shouldnt be possible but I have already heard before someone mentioned this problem so maybe it happened, I had heard it when the site was changed.
Yeah, It ever happened to my recommend lists so I checked back and found that.
The manga was duplicate then someone reported and the admin deleted the one I put in list out.
So when I found the duplicate manga, I will wait till the admin delete before put the left one into list.
As Kyo says, when a duplicate upload is deleted, the record can disappear from your records. It has happened to me, too.

Is it possible that mangas get deleted out of your list without you knowing? Because there are many times that I could have sworn that I had read a yaoi before, but it is not on list.
PS: whenever I finish a manga, I would always put them on the list, so it's not possible that I forget
